The Department of Medicine is a major department, offering a wide range of services.

The departmental mission and vision goes with the MNGHA mission and vision.

It is the primary goal of the Department of Medicine to ensure that every possible effort is directed to fulfill its role in meeting the mission of the Ministry of National Guard Health Affairs in providing National Guard personnel and their eligible dependents as well as all other eligible patients with the highest quality of safe tertiary healthcare while ensuring efficacy and proper utilization of resources. Equally important to the mission are components related to medical and health education and the scientific research.

It is the vision of the Departmental of Medicine to create a professional standard where the highest caliber medical staff are recruited and retained, and up-to-date medical equipment are purchased and maintained while the appropriate rules and ambitious plans are set to ensure the department of Medicine provides patients with nothing but the best available practice, residents with the best attainable training, and employees with the best possible working environment.

Medicine Residency Training Program

Medicine Residency Training Program at King Abdulaziz Medical City-Riyadh, Ministry of National Guard Health Affairs is considered as of one the most excellent and effective programs in the Region.

The Training Program was implemented in 1986 and is being conducted in the Department of Medicine, where residents have the opportunity of learning and working in a tertiary care center for four years. The training is multi-disciplinary approach, follows the principles of evidence-based medicine and CanMEDS (Canadian Medical Education for Directors and Specialists) Competencies standard criteria (e.g., medical expert, professional, scholar, manager, communicator, collaborator, health advocate).

The Resident progresses from a Junior Resident in the first two years to a Senior Resident in the final two years, where he/she acquires more knowledge, leadership and supervisory skills. The final year allows the Residents more autonomy and freedom in assembly leadership role.

The training is supervised by well qualified consultants with North American and European degrees. Most of the faculties have international and local academic experience. The Resident will have the opportunity to spend approximately 18 months in the Clinical Teaching Units/General Medicine and the rest in all medical specialties and critical areas.

As a result of this comprehensive training environment, residents from King Abdulaziz Medical City have always obtained some of the best pass marks and high scores in their examinations. Almost all the Residents who were enrolled in the program became certified in Internal Medicine by at least one Medical Board.

The Residency Training process is directed by the Medicine Residency Training Program Committee.

Department Capacity

Number of trainees is ever increasing over the years adding 30 new R1 residents every academic year and currently we have no less than 110 medical residents excluding board eligible residents.

Mission Medicine Residency Training Program

The main mission of the program is to graduate physicians that can provide comprehensive medical care at a consultant level with good understanding of related cultural, socio-economic, enthical, occupational, environmental and behavioral issues and respecting Islamic religion.

Vision of Medicine Residency Training Program

  • To produce a safe and competent physicians with broad knowledge in the different fields of internal medicine.
  • To provide the residents with enough training so that he/she acquires knowledge, leadership, supervisory skills and more autonomy and freedom in assembly leadership role.
  • To provide an educational environment that promotes the standard of delivery of health care, by appreciation of the logical use of guidelines, text, literature, latest information technology and others.
  • To train residents in learning scholarly character and attitude by giving them assignments to research and present topics of important and relevant medical issues.
  • To prepare residents for future sub-specialty training.
  • To train residents to be collaborators, a health advocate and medical professional in all aspects of residency training over the 4 years of training.

Divisions

Division of Nephrology

Ten Consultant Nephrologist providing expert care in Renal diseases and Allied conditions, in and outpatients including dialysis units. Multidisciplinary care with the exchange of information with various experts through patient oriented conferences. Regional hospital with full laboratory and radiology facility, serving many patients. Supplied via medical school with students, interns, with residency training programs and Nephrology fellowship program.

Teaching basic and clinical Nephrology to fellows, Residents, Interns, Students and nurses including basic, Clinical, Laboratory, Radiological, Clinical pharmacology of Renal drugs and dealing with dialysis machines and patients. Continuing education via textbooks, Journal clubs and ongoing teaching bedside. Rounds, Morning reports and teaching patient care rounds. Preparation Candidates for MD Arab Board and Saudi Board exams. Participating in Resident Selection and MD examinations. Actively participating in General Internal Medicine, Patient care and teaching. Fellowship Training program in Nephrology. Scope of Services

The Nephrology section has ten Consultants and one associate consultant providing care to approximately average over inpatients with renal diseases. The section services approximately —outpatients per month in Nephrology and —- in the dialysis units including peritoneal dialysis.

Infectious Diseases Division

The division delivers high-quality patients care within the mission, vision, and objectives of the Ministry of National Guard Affairs. It supports medical education and training and promotes researches and community health care. The Division of Infectious Diseases in KAMC- Riyadh provides around-the-clock inpatient care, including four infectious disease consultation services to Intensive care unit patients, solid organ transplant, bone marrow transplant, hematological malignancy, and other medical and surgical services, as well as outpatient care.

The infectious disease division service includes diagnosis and management of an extensive range of infections for both immunocompetent and immunocompromised populations, caring for thousands of patients annually. Our group specializes in transplant-associated infection, HIV/AIDS and antimicrobial resistant infections.

Infectious Diseases Department Capacity

Infectious Diseases (ID) division provide inpatient consultation service. More than 5,000 patients seen in 2020 by 4 consultation teams, each is divided to 2 subunits. In addition to the night call to cover consultation and referrals in KAMC, KASCH and different primary health care centers around the clock. Active patients in the ID list is 80-120 per day as per the annual ID report in 2020.

Outpatient Clinics: 10 clinics covering general Infectious diseases and specialty clinics (Transplant ID, HIV). More than 3000 patients referral in 2019. During COVID-19 pandemic, virtual clinics have been activated to provide patients care and ID service. See the ID referral criteria.

Inpatient Service: People living with HIV infection. Hospitalized patients with COVID-19 infections during the early stage of pandemic (March-August 2020)

Antimicrobial Stewardship/Utilization team: Automatic referrals for Antibiotics co-sign (1499 referrals Jan-Sep 2021), Home IV Antibiotics referral (214 referrals in Jan-Sept 2021). Weekly rounds in conjunction with Department of Control and Prevention in the clinical area where demand and use of antimicrobial agents is high.

Division of Endocrinology

The Division of Endocrinology, Department of Medicine is composed of 10 highly qualified consultants with various subspecialty interests. The services provide outpatients, admissions, consults services, education and research. Endocrinology service serving up to 28,000 patients a year as per official statistics in 2020 including 10,000 patients seen with diabetic educators.

The disease pathology is diverse that includes but not limited to complicated diabetes, thyroid cancer, pituitary adenomas, adrenal masses, metabolic bone diseases amongst others. The division provides high quality services administered in subspecialty focused clinics like insulin pump, thyroid cancer, pituitary, lipid, osteoporosis, calcium and metabolism and diabetes clinics. Each consultant is conducting 4 clinics a week.

The division is providing extensive training in endocrinology and diabetes as a major site for SCFHS accredited Fellowship Training Programs in the country. More than tens of graduated fellows have already graduated from our centre and are now full consultants serving the country in various regions.

Research is of prime importance for the division with several published publications every year. It is focus of involvement and education for junior staff, fellows, residents and students.

The Division welcomes all residents and rotating Fellows and rotating residents from other institutions, who take one or maximum of 2 to 3 months endocrinology rotation during their training program.

The Division of Endocrinology & Metabolism provides excellent inpatient and outpatient care to patients with variable endocrine disorders. Our endocrinology specialists are experts in the latest diagnostic and therapeutic approaches offered both in Endocrinology and in Diabetes.

Division of Neurology

The neurology division at King Abdulaziz Medical City (KAMC) provides comprehensive neurological services, engages in undergraduate and postgraduate teaching & training and actively participate in research.

There are currently 14, well qualified, full time consultants plus 2 university consultant. All consultant neurologists have subspecialty training and/or interests in a specific neurology sub-specialty area (cerebrovascular diseases, clinical neurophysiology, epilepsy, movement disorders, neuromuscular disorders and/or sleep medicine). Majority of the full time consultants are affiliated with King Saud Bin Abdulaziz University for Health Sciences (KSAU-HS) with 1 staff at associate professor level, 1 full professor and the rest are assistant professors. The Neurology Residency Training Program (NRTP) currently consists of 30 neurology resident. Our NRTP is approved/accredited by the SCHS to recruit 7 new residents each year. We also have Stroke Fellowship Program and newly approved Clinical Neurophysiology Service Fellowship Program.

The division members provide comprehensive in-patient neurology and stroke services, consultation service, epilepsy monitoring unit assessment and intra-operative monitoring. Other related procedures include Botox therapy as well as different neurophysiological procedures such as electroencephalograms (EEG), evoked potentials (EP), nerve conduction studies (NCS), repetitive nerve stimulation (RNS) and electromyography (EMG) studies are performed on routine basis.

Division of Neurology Capacity

The neurology division staff do total of 60 service months per year. These are divided as 12 months in in-patient neurology unit, 12 months stroke unit, 24 months neurology consultation (2 teams in each month) and 12 months in epilepsy monitoring unit and EMG/EEG service. Plus, Clinic 403 Infusion coverage.

We have approximately 1200 admissions to neurology and stroke units yearly. The average daily number of new consultations was 15. The neurophysiology procedures i.e. nerve conduction studies, electromyography and electroencephalograms are covered by the appropriate staff on weekly rotation basis.

There are in-patient neurological and stroke beds in ward 23 plus there are access beds, as appropriate, in other wards including those in the neuro-intensive care unit (NICU). Easy access to state of the art interventional neuroradiology is available 24/7. Advanced radiology services are easily available at all times. Finally in-patient rehabilitation is available as per protocol.

Division of Pulmonology

Division of Pulmonology provides a wide range of therapeutic, diagnostic, technical and education services to all individuals under the umbrella of King Abdulaziz Medical City, Ministry of National Guard Health Affairs. Our vision is to establish innovative standard of state of the art pulmonary care. That keeps abreast of all new changes in the field of pulmonary medicine and adopt it in our institution for the benefit our patients. Also establish strong educational and training opportunities for our, students, residents, fellows and health care professionals. To establish a very strong subspecialties clinics and protocols with clinical indicators to assess our delivered care. To conduct research in respiratory disease aimed to study respiratory disease/issues related to our community and to translate new development into both daily clinical practice and to rich training environment for the young physician at different level of training and subspecialties.

Division of Pulmonology Capacity

The Pulmonary Service in pulmonary ward (Ward 8) has capacity of 20 beds.

The Pulmonary service is one of the busiest providing consultation services of average 10-15 consultations in a day covering ER, Medical Wards, Surgical Wards, ICU, Cardiac and KASCH.

The Pulmonary section members actively follow and manage all pulmonary patients enrolled in Home Care Program.

The Pulmonary Service attends to KAMC chronic ventilated patients in other facilities (Al Hayyat National Hospital).

Pulmonary Screening Clinic

Pulmonary Physiology Laboratory – well equipped with all required necessary procedures in any tertiary care facilities. Our laboratory is equipped with Full Pulmonary Function laboratory, walking pulse oximetry tests, and ABG facilities. These tests and procedures are available for referrals from other services. Our laboratory is also equipped with other sophisticated procedures, these test includes; methacholine provocation test, exercise induced asthma procedures, overnight pulse oximetry studies, and cardio-pulmonary exercise test.

Sleep Disorders Center - Currently, we have only one bed for sleep study. All our sleep technicians are certified polysomnographist. We perform average 5-6 overnight sleep study per week and 1-2 daytime sleep study per week. Active home sleep study program.

Pulmonary Rehabilitations – The first Pulmonary Rehabilitation established in the Kingdom. There is concise and updated policy and procedures about all the procedures and the process of pulmonary rehabilitation. The pulmonary rehabilitation service covers both inpatients and outpatients pulmonary cases with chronic pulmonary disease.

Bronchoscopy and interventional u/s bronchoscopy – Pulmonary physicians are privileged to perform Bronchoscopy procedures.

Chronic Ventilated Patients – this service covers other service and ICU patients.

Outside KAMC Facility team for Chronic ventilated patients – weekly visit by multidisciplinary team headed by Pulmonary consultant with representatives from Nursing Services, Respiratory Services, and Rehabilitation Services.

Seven Consultant Rheumatologists providing expert care in Rheumatic diseases and allied conditions, in and outpatients, Consultants for patients and referring physicians, and also Consultations to the physiatrists, physical and occupational therapists and orthopedic surgeons.

Continuing education via textbooks, journal clubs and ongoing teaching bedside. Rounds, morning reports and teaching patient care rounds. Preparing Candidates for MD Arab Board and Saudi Board exams. Participating in Residents Selection and MD examinations. Actively participating in General Internal Medicine, patient care and teaching. Fellowship Training Program in Rheumatology. Rheumatology Scope of Service

The Rheumatology section has seven Consultants and one associate consultant provide care to approximately average over 250 inpatients with rheumatic diseases. The Section serves approximately 900 outpatients per month in Rheumatology.

Rheumatology Fellowship

The division started the program in October 2001. It is 2 years training program and our division is also a part of Saudi Board for Rheumatology teaching and fellowship. Since 2008 our training program became a part of Saudi joint Rheumatology fellowship training program that supervised and regulated by Saudi commission of health specialties.

Division of Gastroenterology

Six Consultant Gastroenterologist providing an expert care in Gastroenterology diseases and allied conditions, in and outpatients. Each consultant has 2 clinics per week & 2 endoscopy sessions. IBD Clinic. Supplied via medical school with students, interns, with residency training programs and Gastroenterology Fellowship program.

Teaching basic and clinical Gastroenterology to Fellows, students, interns, residents, nurses and therapist. Continuing education through textbooks, journal clubs and ongoingbedside teaching. Activities include patient care rounds, morning reports, and teaching rounds. The division also organizes GI Grand Rounds, Gastroenterology Radiology Sessions, and Gastroenterology Pathology Sessions. Participating in Residents Selection and MD examinations. Actively participating in General Internal Medicine, patient care and teaching. Fellowship Training Program in Gastroenterology. Some of division members are Assistant Professors in King Saud Bin Abdulaziz University for Health Sciences and participate in lectures, tutorials, student selection and examination.

Division of Gastroenterology Scope of Service

The Gastroenterology section has six Consultants and 4 Associate Consultants also, one Assistant Consultant & one staff physician besides 3 technicians and 12 Nurses, provide care to approximately average over 250 inpatients with diseases of Gastrointestinal Tract. The Section serves approximately 800 outpatients per month in Gastroenterology. The division of Gastroenterology performing more than 40 procedures delay.

Division of Geriatric Medicine

The Division of Geriatric Medicine at NGHA was established in January 2006. The division continues to grow, with a mission that remains the same to become the Center of Excellence in the region in providing a comprehensive care for older adults and for training the future Geriatricians. Today, the division provides care for the inpatient and outpatient older adults population through an inpatient Complex Discharge Unit (24) with 20 beds and ten outpatient clinics, including seven general geriatrics and three specialties for dementia and falls. Our outstanding faculty provide high-quality, patient centered care and a multidisciplinary approach to all older adults and their families to maintain their function and wellbeing in the community.

Department Geriatric Medicine Capacity

  • Inpatient unit (Complex Discharge Ward) with a capacity of 20 beds.
  • Inpatient Consultation
  • Outpatient clinics – 12 clinics and two more clinics will open soon – total of 12 clinics per week (9 geriatrics)
  • Transitional Care – To manage medically stable patients that require a prolonged hospital stay for medical reasons (e.g., I.V. antibiotic, Geriatric Rehab, infected bedsores, etc.)
  • Long Term Care – To supervise the KAMC-Riyadh long-term patients and chronic ventilated patients at Riyadh/Care National Hospital (RNH/CNH) and Riyadh Care Hospital (RCH).
