  • choose the visit reason.

  • fill the input field with the message you want the care giver to receive.

  • An active medical file that is eligible to get the service in one of the Ministry of National Guard Health Affairs facilities or National Guard health centers.

  • The patient must be referred to the home health department by the doctor responsible for the patient.

  • National ID or Iqama number
